Do you need the original source file, or can you work from a scan/photo?
Either works. A source file (Word, etc.) gives the cleanest result, but I can also rebuild a form from a scanned image or clear photo.
What software do you use to build the forms?
I use professional PDF authoring tools (Adobe Acrobat) to ensure the forms are compatible with standard PDF readers.
