A healthcare clinic needed a 24/7 AI assistant capable of handling patient inquiries, booking appointments, and answering common questions — across Telegram, WhatsApp, and SMS simultaneously. Their team was manually responding to hundreds of messages daily across three platforms with no central system, causing missed inquiries and delayed bookings. I built a fully automated multi-channel AI assistant using n8n. The system triggers from both Telegram and Twilio, routes messages through a smart router, and handles five auto-message flows including start, contact, how-it-works, limitations, and live data from Google Sheets. A Redis buffer aggregates rapid incoming messages before passing them to the AI brain — preventing duplicate processing and reducing API costs significantly. The core AI agent (Anna) runs on OpenAI with persistent conversation memory, Gmail tool access, and client info lookup. Anna connects to a Google Sheets company agent and a calendar system covering 4 doctors, each managed by a dedicated OpenAI model. Replies are delivered back across all three channels — SMS via Twilio, Telegram, and WhatsApp — automatically and in real time. Result: zero missed inquiries, 90% reduction in manual response time, and fully automated appointment booking across all channels.
