As an example of the custom side, I built a full Hospital Management module from scratch as a portfolio project, covering custom models, views, and business logic end to end happy to walk you through it.

I have a full-stack development background (C#/.NET, Python) and two years of production software experience at NHS Wales, so I'm used to building things that have to actually work reliably for real users, not just demo well.
