Have a printable form that LOOKS as if it was done haphazardly? Do lines get cutoff or print in the wrong area of the page? Do sections not line up properly? Would you like to fit more information on one page? Let me re-do your form for you & provide you with a clean, crisp printable form. Will give it to you in Word & PDF. A single gig includes up to 2 pages, one-sided.  Additional pages will require additional gigs unless you contact me to negotiate otherwise. Send me your form in Word, Publisher, RTF, Text or PDF format. If you do not have a form, please provide a text document of the exact phrases and questions you want on the form as well as a drawn sketch of how you want the form to appear & where the information should go. Or, find an image of a form that is in a format that you like (images.google.com). The complexity of the form will determine actual delivery date.  The more complex - the longer it will take.  Will also negotiate delivery date if you absolutely need it sooner.
